Work Environment

This role is an onsite position in a lively assisted living facility that promotes a collaborative culture among medical professionals. The clinic operates Monday to Friday from 9 AM to 5 PM, allowing for a predictable work schedule that supports work-life balance. You will be joining a team of compassionate and experienced professionals dedicated to providing exemplary care and support to our residents.

Key Responsibilities

• Conduct comprehensive assessments of patients, including their health history, physical examinations, and psychological evaluations to develop appropriate treatment plans.
• Provide ongoing diagnosis, treatment, and consultation for a variety of acute and chronic health issues in alignment with best practices and guidelines of internal medicine.
• Engage in disease prevention and health promotion by addressing lifestyle choices, educating patients on risk factors, and developing individualized health education plans.
• Collaborate closely with physicians, nurses, and other healthcare professionals to discuss patient care strategies, including necessary referrals and follow-ups for specialized services.
• Maintain thorough and accurate patient records, utilizing electronic health record (EHR) systems for documentation and communication of patient care plans and progress.
• Deliver patient education to individuals and families regarding medical conditions, treatment options, preventive care, and the proper use of medications.

Patient Population / Client Focus

Your primary focus will be on providing healthcare services to the assisted living community, which comprises a diverse population of residents predominantly aged over 65. This role requires a deep understanding of the complexities of managing chronic diseases, geriatric care, and the particular emotional and social needs of older adults.

Qualifications

• A Bachelor’s Degree in Nursing from an accredited university.
• A current and valid Nurse Practitioner license or eligibility to practice in Maryland, with national certification as a Family Nurse Practitioner (FNP).
• Experience working with geriatric populations is highly valued.
• Acute Care certified NPs will not be considered for this role.

Preferred Experience

Ideal candidates will have at least two years of direct nursing experience, with a focus on internal medicine or geriatric care. Candidates should possess strong critical thinking and interpersonal skills, as well as the ability to work both independently and collaboratively within a multidisciplinary team.

Compensation and Benefits
