Key Responsibilities

• Conduct thorough patient assessments, including physical examinations and health history reviews, to accurately diagnose acute and chronic health issues.
• Formulate and implement individualized evidence-based treatment plans, working in close coordination with collaborating physicians and healthcare teams.
• Prescribe medications and other therapies while managing medication adherence, adjustments, and potential side effects during follow-ups.
• Order, interpret, and follow up on necessary diagnostic tests, such as blood work and imaging, leveraging advanced healthcare technology.
• Educate patients and their families on health management, emphasizing preventive care, managing existing conditions, lifestyle modifications, and post-visit care.
• Engage in interdisciplinary rounds, contributing your insights to patient discussions and ensuring seamless continuity of care.
• Utilize electronic medical records to maintain accurate, up-to-date documentation and patient history, streamlining care processes.

Patient Population / Client Focus

This role focuses on male and female patients of all age groups, including those with both chronic and acute health needs. Qualifications

• Current certification as a Nurse Practitioner and an active Maryland nursing license.
• A Bachelor's degree in Nursing, with a preference for candidates holding a Master's degree in nursing from an accredited institution.
• A minimum of four years of clinical experience, preferably in a primary care or concierge medicine setting.
• Exceptional communication and collaboration skills to effectively work within a multidisciplinary team.

Compensation and Benefits
