The Gundersen Health and Wellness Clinic is excited to announce a full-time opportunity for a dedicated Nurse Practitioner (NP) to join our Center for Advanced Cardiac Care (CACC) team in beautiful Mineola, New York. In this pivotal role, you will play a vital part in delivering high-quality cardiovascular care to our outpatient population, working under the supervision of the Chief Nurse Practitioner and in collaboration with our esteemed CACC Medical Director. This position ensures patient care is holistic and comprehensive, helping to improve overall health outcomes through expert medical interventions and patient education.

Work Environment

This role primarily operates in an onsite Assisted Living Facility within the community, providing a supportive and collaborative atmosphere. You will be part of a multidisciplinary team, collaborating daily with Physicians, fellow Nurse Practitioners, Registered Nurses, Social Workers, Nutritionists, and other health professionals to ensure a seamless continuum of care. Our modern facility is equipped with advanced medical technologies, fostering an environment of learning and patient care excellence.

Key Responsibilities

• Conduct comprehensive patient assessments by obtaining medical histories and performing targeted physical examinations to identify health concerns.
• Order and interpret diagnostic tests, effectively diagnosing and managing both chronic and acute patient conditions.
• Develop and implement individualized care plans, encompassing health maintenance, anticipatory guidance, and appropriate medical interventions informed by established protocols.
• Regularly engage in collaborative follow-ups with physicians to adjust and optimize treatment strategies based on patient's progress.
• Educate and counsel patients and caregivers on disease management and health improvement strategies, facilitating post-hospital care.
• Coordinate with the inpatient care team to facilitate safe patient admissions and discharges, ensuring continuity in care.
• Effectively triage and manage patient inquiries through daily calls, coordinating with referring providers when required.
• Document all patient interactions thoroughly and accurately within electronic medical records (EMR) to maintain up-to-date patient history.
• Engage in clinical research initiatives, working to incorporate the latest findings into practical, evidence-based care.
• Maintain a safe and therapeutic environment conducive to patient recovery and well-being.

Patient Population / Client Focus

The Nurse Practitioner will focus primarily on an adult population within the cardiovascular care spectrum, including patients with advanced Heart Failure, Pulmonary Arterial Hypertension, and those undergoing mechanical circulatory support therapies. The NP will play an essential role in managing patients post-heart transplant and collaborating with specialists to ensure comprehensive care tailored to each patient's unique needs.

Qualifications

• Master's Degree in Nursing with specialized training as a Nurse Practitioner in adult care.
• Current New York State Registered Professional Nurse and Nurse Practitioner licenses, along with NP Board Certification.
• Active certifications in Basic Life Support (BCLS), Advanced Cardiac Life Support (ACLS), DEA, and NPI.
• Proficiency in electronic medical record systems and common software tools, including MS Word, Excel, and PowerPoint.

Preferred Experience

Applicants with at least two years of experience working in a hospital or outpatient setting, ideally within cardiology, will be preferred. Experience managing complex patient cases and familiarity with advanced therapies will be beneficial. We value candidates who demonstrate expertise in caring for populations with specific cardiovascular needs and those interested in furthering their skills through continuing education.

Compensation and Benefits

This position offers a competitive salary ranging from $120,000 to $220,000 per year, commensurate with your experience and qualifications.
