Key Responsibilities

As a Nurse Practitioner in our Cardiology Clinic, your responsibilities will include:

• Conducting thorough patient histories and performing comprehensive and targeted physical examinations to evaluate health concerns.
• Ordering and interpreting diagnostic tests and utilizing clinical findings to diagnose and manage both chronic and acute health issues.
• Formulating and implementing goal-directed care plans that encompass health maintenance, medical interventions, and anticipatory guidance in collaboration with the healthcare team.
• Reviewing patient records and reports from external sources to ensure a holistic understanding of each patient’s health status.
• Providing patient education and disease management advice, empowering patients and caregivers to take active roles in their health and wellness.
• Coordinating with inpatient teams for hospital admissions and facilitating safe discharges to enhance continuity of care.
• Documenting all interactions and patient encounters within our electronic medical records system, ensuring accuracy and compliance with regulatory standards.
• Participating in research protocols and incorporating research findings into clinical practice to advance cardiac health care.
• Contributing to creating a safe and therapeutic environment for patients while demonstrating accountability for all aspects of Nurse Practitioner practice.

Patient Population / Client Focus

In this role, you will primarily work with adult patients suffering from a variety of cardiac conditions, including heart failure and post-transplant care. Your expertise will be utilized in managing cases involving advanced therapies, including outpatient intravenous medications and mechanical circulatory support. You will be instrumental in closely monitoring patient progress and adapting care strategies as needed, ensuring optimal patient outcomes in a fast-paced healthcare setting.

Qualifications

The ideal candidate for this position will have:

• A Master's degree in Nursing with specialized training as a Nurse Practitioner focusing on adult patients.
• A minimum of two years of clinical experience in a hospital or outpatient setting, with a strong preference for experience in cardiology.
• Current licensure as a Registered Professional Nurse and Nurse Practitioner in New York State, along with NP Board certification.
• Certifications in Basic Life Support (BCLS), Advanced Cardiovascular Life Support (ACLS), as well as DEA and NPI credentials.
• Proficiency with electronic medical records systems and standard office computer applications such as MS Word, PowerPoint, and Excel.

Preferred Experience

Preference will be given to candidates who have:

• Significant exposure to complex cardiac patient populations, particularly those dealing with advanced heart failure and pulmonary arterial hypertension.
• Hands-on experience with patients undergoing advanced therapies and those requiring comprehensive outpatient care plans.

Compensation and Benefits
