Full-Time Nurse Practitioner (NP) - School-Based Care

Position Overview

Gotham Enterprises Ltd is seeking an experienced and dedicated Nurse Practitioner (NP) to join our dynamic healthcare team in a school-based setting located in Valley Stream, New York. This full-time role requires a focus on providing exceptional primary care to adolescents and young adults, ensuring they receive the integrated support needed to thrive academically and personally.

Work Environment

This position will be based onsite at local schools within the Valley Stream area. You will be part of a collaborative outpatient care team comprising physicians, physician assistants, medical assistants, and other support staff. Each member plays a crucial role, working together to ensure that both immediate and long-term health needs of students are met in a nurturing and supportive educational environment.

Key Responsibilities

• Provide exceptional primary care services, assisting students with both routine check-ups and acute health issues.
• Conduct annual wellness visits and thorough health risk assessments, emphasizing a holistic approach to student health.
• Facilitate effective care coordination among various healthcare professionals, ensuring comprehensive support for students.
• Lead population health initiatives that promote preventive care measures, like vaccinations and screenings, among the student population.
• Educate students and their families on health conditions, treatment options, and preventive care practices tailored to their unique needs.
• Participate in school wellness programs, working closely with school administration to enhance student health outcomes.
• Complete accurate and timely documentation as it relates to patient care and follow-ups, critical for maintaining high-quality care standards.
• Assist the care team in triaging calls or messages from parents and guardians, ensuring timely responses to health concerns.
• Occasionally perform home visits as necessary to provide care or support for students with significant health needs.
• Keep open lines of communication with the Medical Director and operational leadership to ensure effective clinic management.

Patient Population / Client Focus

Our focus is on providing personalized care for students and young adults, with an emphasis on preventative health practices, early intervention, and maintaining optimal health. Many of our patients are from diverse backgrounds and may face specific socioeconomic challenges, requiring a compassionate and culturally competent approach to healthcare delivery.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or a relevant Nurse Practitioner Certificate Program.
• National certification from AANP or ANCC as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner.
• A valid, non-probationary state Nurse Practitioner license, along with an active DEA license.
• Prior experience in primary care settings such as internal medicine, geriatrics, or family medicine is preferred.
• A solid understanding of HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
• Exemplary interpersonal skills with a passion for teamwork, patient education, and collaborative healthcare delivery.
• Bilingual proficiency is a plus, enhancing communication with diverse patient populations.

Preferred Experience

• Experience working in school-based health services or community health organizations is highly desirable.
• Familiarity with electronic medical records (EMR) systems and patient care coordination tools.
• Demonstrated ability to engage with students, families, and faculty to foster collaborative health initiatives.

Compensation and Benefits

The role offers a competitive annual salary ranging from USD 120,000 to 160,000, based on experience and expertise. We also provide an array of benefits including:
