Work Environment

This position is located onsite at our facility in Valley Stream, NY, with occasional home visits for continuity of care. You will be part of an interdisciplinary team that prioritizes a collaborative approach, including physicians, other nurse practitioners, physician assistants, and dedicated support staff. Key Responsibilities

• Provide exceptional primary care services, including both routine and acute care consultations.
• Conduct comprehensive annual wellness visits and health risk assessments, ensuring thorough and precise documentation tailored to individual patient needs.
• Coordinate care with various healthcare providers, engage with specialists, and communicate with testing facilities to streamline patient treatment pathways.
• Manage population health initiatives in collaboration with the care team, ensuring that patients meet crucial screening and preventative care benchmarks.
• Participate actively in patient education, guiding individuals on health conditions, treatment plans, and promoting overall wellness.
• Assist with phone triage, outreach efforts, and community engagement activities to bolster patient access to care.
• If needed, conduct home visits to provide personalized care in the comfort of patients' homes.
• Engage in team discussions, reporting to the Medical Director, while contributing to the collective operational and clinical objectives of the organization.

Patient Population / Client Focus

Your primary focus will be on adults who are part of the Medicare demographic, typically facing a range of health complexities associated with aging. These individuals rely on our organization not only for medical expertise but also for a healthcare provider who understands the nuances of geriatric care. By fostering strong relationships with your patients, you will cultivate trust and enhance their engagement in their care plans, ultimately promoting better health outcomes.

Qualifications

• A Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of a Nurse Practitioner program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through a recognized body (AANP or ANCC).
• Possession of a current, non-probationary Nurse Practitioner state license.
• Active Drug Enforcement Administration (DEA) license.
• Experience in primary care settings, with a focus on internal medicine, geriatrics, or family medicine.
• Understanding of HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
• A collaborative mindset with a passion for working in multifunctional teams to achieve optimal patient care.

Preferred Experience

• Previous experience in outpatient primary care settings is highly regarded.
• Familiarity with electronic health record (EHR) systems and documentation practices.
• Background in health promotion and disease prevention methodologies.
• Experience in care transitions and chronic disease management.

Compensation and Benefits

Riverhead Physical Medicine and Rehabilitation offers a competitive salary range of USD 120,000 to 160,000 annually, commensurate with experience and qualifications.
