Key Responsibilities

• Develop and implement targeted health education programs in partnership with local organizations and stakeholders, aiming to enhance health literacy within the veteran population.

• Conduct comprehensive assessments, including physical examinations and health histories, to develop personalized care plans in line with established clinical guidelines.
• Maintain detailed and accurate medical records using our electronic health record (EHR) system, ensuring compliance with local, state, and federal regulations.
• Participate in community outreach initiatives, including health fairs and workshops, to facilitate screenings, preventive care, and health promotion.
• Stay informed on community health trends, evidence-based practices, and policies relevant to the care of veterans.

Patient Population / Client Focus

This position primarily focuses on serving veterans and their families, who may face various health challenges, including chronic conditions, mental health issues, and social determinants of health. Your expertise will be crucial in addressing these unique needs through both direct patient care and community-based initiatives.

Qualifications

• A Master’s or Doctoral degree in Nursing from an accredited program is required.
• Current certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from reputable accrediting bodies such as ANCC or AANP.
• A valid and unrestricted Nurse Practitioner (APRN) license in New York State is necessary.
• Possession of a DEA# and NPI# is required.
• Exceptional clinical assessment and diagnostic skills to effectively evaluate patient health and needs.

Preferred Experience

• A minimum of 3–5 years of clinical nursing experience is preferred, with a focus on community health or public health settings.
• Proven ability to work effectively with diverse populations, demonstrating strong interpersonal and communication skills.
