Work Environment

This position is onsite at our assisted living facility, providing an interactive and collaborative atmosphere. The typical work schedule will be Monday through Friday from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Participation in community engagement activities, such as health fairs or outreach events during weekends, is also expected. The role encompasses an average workweek of approximately 40 hours, aligning with our commitment to community health.

Key Responsibilities

• Perform thorough health assessments on individuals and families to identify healthcare needs and establish care plans tailored to each patient's unique situation.
• Diagnose and manage acute and chronic medical conditions, providing preventive care and health promotion tailored to the community's needs.
• Collaborate with local organizations to design and implement comprehensive health education programs, enhancing community awareness about vital health issues.
• Advocate for culturally sensitive care, ensuring that all patients have equal access to healthcare services.
• Coordinate referrals and manage follow-up care to maintain continuity of care, ensuring patients receive the necessary resources and support.

Qualifications

Compensation and Benefits

The Nurse Practitioner position offers a competitive salary ranging from $180,000 to $200,000 per year, commensurate with experience and qualifications.
