Work Environment

This position is based at a school setting in Lynbrook, ensuring that you engage directly with children and families in their formative environments. Key Responsibilities

• Conduct comprehensive health assessments and screenings, tailoring your approach to meet the needs of diverse populations.
• Diagnose and manage both acute and chronic medical conditions, prescribing tailored treatment plans that emphasize prevention and health promotion.
• Collaborate with local community organizations to design and implement health education programs, focusing on preventative measures and self-care tactics.
• Advocate for patients’ rights, ensuring equitable access to healthcare services while providing culturally appropriate care.
• Coordinate referrals and follow-up care to guarantee continuity, maintaining consistent care pathways for patients.
• Prepare meticulous medical records that comply with regulatory standards, ensuring proper documentation and effective communication within health teams.
• Engage actively in community outreach initiatives and health fairs, utilizing these platforms to raise awareness about health issues and offer essential screenings.
• Remain informed of the latest trends in community health, evidence-based practices, and emerging policies that impact the populations we serve.

Patient Population / Client Focus

You will primarily work with children, adolescents, and their families in an educational setting, addressing a wide array of health concerns that may affect their daily lives and academic performance. Your role will be pivotal in identifying health risks early, providing preventive care strategies, and engaging families in a collaborative healthcare approach.

Qualifications

• A Master’s or Doctoral degree in Nursing from an accredited institution.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from relevant accrediting bodies.
• Current, unrestricted Nurse Practitioner license in New York State.
• Possession of DEA# and NPI# is required.

Preferred Experience

• At least 3-5 years of clinical experience, ideally within primary care or community health settings.
• Demonstrated ability to foster trusting relationships with patients and collaborate effectively with multidisciplinary teams.
• Strong clinical assessment, diagnostic skills, and adeptness with electronic health record (EHR) systems.
• Experience in conducting community outreach for health promotion is advantageous.

Compensation and Benefits

Your dedication to improving community health and well-being will be rewarded with a competitive salary ranging from USD 150,000 to 160,000 annually. Additionally, a sign-on bonus of up to $10,000 is available to successful candidates.
