Work Environment

The Nurse Practitioner will operate onsite at an assisted living facility, a vital aspect of our community health initiative. Key Responsibilities

• Conduct thorough health assessments, including cardiovascular screenings and evaluations, tailored to the needs of each patient.
• Diagnose and develop treatment plans for acute and chronic medical conditions, with an emphasis on preventive care and health promotion, particularly in cardiology.
• Collaborate with community organizations to design and implement educational programs that raise awareness about heart health.
• Advocate for patients’ rights and promote access to necessary healthcare services, ensuring culturally competent care.

Patient Population / Client Focus

This role primarily focuses on serving the elderly population and individuals with complex cardiovascular needs in our assisted living facility. Qualifications

• A Master’s degree in Nursing from an accredited program.
• An active nursing license in the state of New York.
• Current certification as a Nurse Practitioner, with a focus on cardiology preferred.

Compensation and Benefits

We offer a competitive salary range of $150,000 to $160,000 per year, reflective of your experience and qualifications.
