Key Responsibilities

• Conduct thorough health assessments, including detailed physical examinations and necessary screenings, catering to a diverse population of residents with varying health conditions.
• Diagnose and manage both acute and chronic medical conditions, with an emphasis on preventive care and health promotion initiatives.
• Collaborate with residents and their families to develop personalized treatment plans, ensuring that the care provided aligns with each individual’s preferences and health goals.
• Advocate for patients' rights, providing culturally sensitive care and ensuring access to necessary healthcare services.
• Coordinate referrals to specialists when needed, along with comprehensive follow-up care to maintain continuity in patient healthcare.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve a diverse patient population within our assisted living facility, addressing the unique needs of older adults, many of whom may be managing multiple chronic conditions. Your role will be crucial in developing specialized care plans that promote wellness, prevent hospitalizations, and enhance the overall health outcomes of our residents.

Qualifications

• Master’s degree in Nursing from an accredited program.
• Current certification as a Nurse Practitioner, with an active New York state nursing license.
• A solid foundation of clinical knowledge and skills, ideally with a focus on geriatric care.
• Exceptional interpersonal and communication abilities to effectively engage with patients, families, and colleagues.

Preferred Experience

Compensation and Benefits

The annual salary for this position ranges from $150,000 to $160,000, commensurate with experience and qualifications.
