Work Environment

This role is set within an engaging onsite facility at a local school, allowing for direct interactions with our patient population in a familiar and comfortable setting. You will be integrated into a collaborative care team, which includes physicians, physician assistants, medical assistants, dedicated medical scribes, and supportive registered nurses and care managers. This structured team dynamic ensures that you can focus on providing high-quality patient care while receiving support for coordination and follow-up.

Key Responsibilities

• Deliver exceptional primary care services that include assessments, diagnoses, and treatment planning for a variety of health concerns.
• Conduct routine office visits for both acute and chronic issues, ensuring thorough evaluations and personalized care plans.
• Perform annual wellness visits and comprehensive health risk assessments, focusing on detailed documentation that informs ongoing patient care.
• Coordinate care with various external entities including specialists, testing facilities, and agencies to optimize patient outcomes.
• Lead efforts in population health initiatives by ensuring adherence to preventative care guidelines, such as timely cancer screenings.
• Assist the care team in managing phone triage, responding to patient inquiries, and initiating follow-up outreach.
• Provide education and counseling to patients about their conditions, treatment options, and overall health management strategies.
• Engage in community outreach and promotional activities to raise awareness of our services and health initiatives.
• Conduct home visits when necessary, providing care for patients who require additional support beyond the clinic.
• Report directly to the Medical Director and collaborate closely with operational leadership to meet organizational goals and enhance service delivery.

Patient Population / Client Focus

Your primary focus will be on adult patients with Medicare, necessitating a deep understanding of their unique healthcare needs. Our patients often seek preventive care to maintain their quality of life, and your expertise will empower them to take charge of their health. By building long-term relationships, you will contribute significantly to the continuity and effectiveness of their care.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or a completion of a Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner (FNP, ANP, or AGNP) via AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license and an active DEA license.
• Proven experience in primary care settings, with a focus on internal medicine, geriatrics, or family medicine preferred.
• Familiarity with HCC documentation, ICD-10 coding guidelines, and proficient in conducting Health Risk Assessments.
• A strong commitment to collaboration and teamwork in a cross-functional healthcare environment.

Preferred Experience

• Experience working in outpatient clinic settings where patient relationships are nurtured for long-term health outcomes.
• Background in utilizing electronic health record systems for efficient documentation and patient management.
• Previous leadership or team-coordination experience beneficial in enhancing a cohesive work environment.

Compensation and Benefits

We offer a competitive salary ranging from USD 120,000 to USD 160,000 annually, commensurate with experience and qualifications.
