Key Responsibilities

• Collaborate with physicians and healthcare professionals to deliver a seamless continuum of care to patients.
• Conduct thorough medical, physical, and psycho-social evaluations, ensuring comprehensive health assessments tailored to individual patient histories and needs.
• Perform preventive screenings and examinations, implementing necessary therapeutic and corrective measures.
• Develop accurate differential diagnoses based on comprehensive assessments and clinical findings, collaborating closely with primary care teams.
• Create and execute tailored patient education plans that empower individuals to understand their health conditions and treatment options.
• Prescribe and manage appropriate pharmacologic and non-pharmacologic therapies to effectively address various medical conditions.

Patient Population / Client Focus

As an Internal Medicine Nurse Practitioner, you will primarily engage with adult and elderly patients grappling with chronic illnesses, complex health conditions, and preventive healthcare needs. You will be responsible for managing patients with multifaceted medical histories and varying degrees of health literacy, allowing for meaningful relationships and impactful patient care.

Qualifications

• A Master's Degree as a Nurse Practitioner.
• An active Registered Nurse (RN) license in the state of New York.
• A minimum of three years of relevant clinical practice experience in an outpatient or primary care setting.
• Proven proficiency in utilizing electronic health record (EHR) systems and medical technology, streamlining patient care and documentation.
