Work Environment

This position is based onsite, allowing you to interact directly with patients in their homes or at our facility. Your work schedule will typically run from 9:00 a.m. to 6:00 p.m., with the possibility of evening clinics until 8:00 p.m. on certain days. Additionally, your involvement in community outreach events may necessitate weekend participation, further strengthening your connection to the Valley Stream community.

Key Responsibilities

As a Nurse Practitioner, you will be responsible for:

• Performing thorough health assessments and screenings for a diverse range of individuals and families.
• Diagnosing and managing both acute and chronic medical conditions, providing preventive care and fostering health promotion strategies tailored to the community's specific needs.
• Collaborating with various community organizations to develop and execute impactful health education initiatives that promote long-term wellness.

Qualifications

Compensation and Benefits
