Work Environment

Located in the heart of Freeport, our onsite facility combines the convenience of traditional healthcare settings with the personalized service expected in concierge medicine. The work schedule typically spans from 9:00 a.m. to 6:00 p.m., with the occasional evening clinic running until 8:00 p.m. This structure supports an average workweek of approximately 40 hours, allowing for a balanced professional and personal life.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, utilizing advanced diagnostic tools including stress tests, chest X-rays, and other necessary diagnostics.
• Manage and treat chronic conditions such as hypertension, diabetes, and high cholesterol, ensuring patients receive appropriate medication management, especially those with congestive heart failure (CHF) and atherosclerosis.
• Diagnose acute and chronic medical conditions, while promoting preventive care and health education to empower patients in their health journeys.
• Collaborate effectively with community organizations to implement health education programs and outreach initiatives, engaging the population in better health practices.
• Provide culturally sensitive care, advocating for patient rights and equitable access to healthcare services.
• Coordinate referrals and follow-up care meticulously to maintain continuity and consistency in patient care.
• Maintain precise and comprehensive medical records to comply with all regulatory standards and improve the quality of care.

Patient Population / Client Focus

This role primarily serves the Freeport community, working with a diverse patient population that includes individuals from various backgrounds and age groups. Qualifications

• A Bachelor’s Degree in Nursing.
• Must be a licensed Nurse Practitioner in the state of New York.

Compensation and Benefits

This position offers a competitive annual salary ranging from USD 120,000 to 140,000, commensurate with experience and qualifications.
