Work Environment

Our work setting is primarily onsite, where you will engage directly with patients in their homes or communities. You will work within a supportive and collaborative team, receiving assistance from administrative staff and collaborating with other health professionals to ensure a seamless patient experience. The work schedule typically spans from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Participation in weekend community outreach events and health fairs may also be required, totaling approximately 40 hours a week.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, identifying physical and mental health needs.
• Diagnose and manage a variety of acute and chronic medical conditions, designing personalized care plans that focus on preventive care and health promotion.
• Work collaboratively with community organizations to develop and implement targeted health education initiatives, workshops, and programs aimed at health literacy.
• Deliver culturally competent care, ensuring that services are accessible and respectful of patient diversity while advocating for patients’ rights in healthcare settings.
• Coordinate referrals and follow-ups to guarantee continuity of care; this includes ensuring that patients understand their treatment plans and are engaged in their healthcare journeys.
• Maintain comprehensive and detailed medical records while adhering to all regulatory standards, contributing to the efficient tracking of patient progress and outcomes.
• Participate in community health events, providing health screenings and consultations that empower community members with knowledge about preventive care.
• Stay informed about current community health trends, evidence-based practices, and relevant public health policies, integrating this knowledge into your daily services.

Qualifications

Compensation and Benefits

This position offers a competitive salary range of $180,000 to $200,000 annually, commensurate with experience and qualifications.
