Work Environment

This position is based onsite within a concierge medicine model, primarily involving house calls. Key Responsibilities

• Autonomously manage a diverse range of patient care scenarios, assessing and diagnosing acute, critical, and chronic health conditions.
• Exercise prescription authority, accurately determining medication dosages and frequencies based on individual patient assessments.
• Collaborate effectively within a multidisciplinary team, leveraging various resources and consultants to enhance patient care.
• Coordinate patient transitions smoothly across different healthcare settings, ensuring optimal communication with all involved healthcare providers.
• Maintain up-to-date electronic patient records, documenting encounters, assessments, treatment plans, and progress, ensuring continuity of care among teams.
• Engage with patients in meaningful discussions regarding health management, contributing to custom-tailored treatment plans and facilitating access to additional care when necessary.

Patient Population / Client Focus

As a Family Nurse Practitioner, you will care for a diverse patient demographic in Long Island City, New York. Your clientele will include individuals from varied socioeconomic backgrounds, some of whom may have limited access to traditional healthcare services. You will play a pivotal role in advocating for these patients, ensuring they receive equitable and compassionate medical attention, regardless of their circumstances.

Qualifications

• Current Nurse Practitioner certification and an active state nursing license are mandatory.
• A Bachelor’s or Master’s degree in Nursing from an accredited institution is required.
• At least three years of clinical experience, preferably within family medicine or a related specialty.
• Exceptional communication and interpersonal skills are essential for building rapport with patients and colleagues.
• Strong analytical and problem-solving abilities to navigate complex patient scenarios effectively.
• Proficiency in both Spanish and English to effectively communicate with our diverse patient population.

Preferred Experience

Compensation and Benefits

The Community Family Wellness Center offers a competitive annual salary ranging from $120,000 to $130,000 , reflecting your expertise and dedication to health care.
