Work Environment

This role is situated onsite at our outpatient office, providing a conducive environment for direct patient interaction. The work hours typically span from 9:00 a.m. to 6:00 p.m., with occasional evening clinics to accommodate patient needs, and an approximate average of 40 hours per week. Participation in weekend community outreach events may also be required, contributing to our mission of public health education.

Key Responsibilities

• Conduct thorough health assessments and screenings, gathering detailed patient histories and physical examinations.
• Diagnose and manage a range of acute and chronic medical conditions with evidence-based approaches.
• Create individualized care plans that promote preventive care, lifestyle changes, and health maintenance.
• Collaborate with community stakeholders to develop and implement educational programs addressing specific health issues.
• Provide culturally sensitive healthcare, ensuring that all patients feel respected and understood while advocating for their rights.
• Coordinate follow-up appointments and referrals to specialists, ensuring continuity of care.

Qualifications

• Current certification as a Nurse Practitioner is required.
• Active nursing license in the state of New York.

Compensation and Benefits
