Work Environment

Located in the vibrant area of Long Island City, our facility provides a supportive and collaborative atmosphere for our healthcare professionals. The office operates primarily on weekdays from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Additionally, you may be asked to participate in community outreach events and health fairs during weekends, contributing further to our mission beyond the walls of the facility. On average, you will work approximately 40 hours per week, engaging directly with a diverse patient population.

Key Responsibilities

• Conduct in-depth health assessments and screenings, identifying both acute and chronic conditions for individuals and families.
• Diagnose and create care management plans tailored to the unique needs of each patient, incorporating preventive care strategies and health promotion activities.
• Collaborate with local health organizations to design and implement impactful health education programs aimed at community well-being.
• Deliver culturally sensitive care to a diverse patient demographic and advocate for their rights in accessing necessary healthcare services.
• Coordinate referrals and ensure appropriate follow-up for continuity of care, addressing each patient's unique circumstances and needs.
• Maintain meticulous and compliant medical records that reflect the highest standards of care.
• Engage in community outreach through health fairs and educational workshops, positively impacting public health awareness.

Patient Population / Client Focus

As a Bilingual Nurse Practitioner, you will primarily serve the residents of our Assisted Living Facility, which accommodates a multicultural population with varying health needs. Your ability to communicate fluently in Spanish will be instrumental in ensuring that all patients receive personalized care that respects their cultural backgrounds. Qualifications

• Current certification as a Nurse Practitioner and active state nursing license is required.

Compensation and Benefits

Little Vista Life Medical, LLC offers a competitive salary range of USD 120,000 to 130,000 annually, commensurate with experience and qualifications.
