Key Responsibilities

• Provide exemplary primary care services by conducting thorough assessments and managing routine and acute health issues in an outpatient setting.
• Perform annual wellness visits and health risk assessments, emphasizing holistic health metrics and thorough documentation for high-quality care delivery.
• Coordinate care with other professionals including specialists and rehabilitation services, ensuring a comprehensive treatment strategy for each patient.
• Lead population health initiatives within the Care Team, including ensuring timely screenings and preventive measures such as evidence-based breast cancer assessments.
• Participate in phone triage to address patient concerns and provide timely outreach for proactive health management.
• Educate patients on their health conditions and treatment plans, fostering a deeper understanding of their ongoing care.
• Contribute to promotional and community engagement activities to extend the reach of our services within the Brooklyn community.
• Conduct home visits when necessary, particularly for patients with mobility challenges or complex medical needs.
• Report directly to the Medical Director and collaborate with operational leadership to continuously improve care delivery.

Patient Population / Client Focus

Your primary focus will be on adult patients, particularly those who are Medicare beneficiaries. Our care model is specifically designed to meet the unique health challenges faced by older adults. You will work to establish meaningful long-term relationships, enhancing their trust and understanding of their health care plan to create a patient-centered experience that aligns with their individual needs.

Qualifications

• A Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program, or a graduate of an NP Certification Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner recognized by either the AANP or ANCC.
• An active, non-probationary Nurse Practitioner license in the state of New York.
• An active DEA license is required.

Preferred Experience

• Prior experience in primary care settings such as internal medicine, geriatrics, or family medicine.
• Familiarity with Health Risk Assessments (HRAs), Hierarchical Condition Categories (HCC) documentation, and ICD-10 coding standards.
• Demonstrated ability to work in a collaborative team environment, with a passion for cross-functional teamwork.
• Bilingual proficiency is desirable to meet the needs of a diverse patient demographic.

Compensation and Benefits

The annual salary for this position ranges from USD 115,000 to 150,000, dependent upon experience and expertise. In addition to competitive compensation, we offer a comprehensive benefits package that includes:
