Next Horizon Health Care is seeking a dedicated and passionate Nurse Practitioner (NP) to join our innovative healthcare team. This full-time position is based in Jersey City, New Jersey, primarily focusing on home care for adults on Medicare. Work Environment

This position requires working onsite in a dynamic home care setting where you will collaborate closely with physicians, nurse practitioners, physician assistants, and support staff, including medical scribes and care managers. Our multidisciplinary team approach allows for effective care coordination, ensuring all aspects of a patient’s health are addressed. The typical work hours are Monday through Friday from 9:00 AM to 5:00 PM, providing a predictable schedule for work-life balance.

Key Responsibilities

• Provide comprehensive primary care to patients, focusing on their unique health needs and situations.
• Conduct thorough office visits that tackle both routine check-ups and acute health issues.
• Administer annual wellness visits and perform health risk assessments, ensuring thoughtful documentation and a patient-centered perspective on health management.
• Coordinate care with various healthcare providers and specialists to ensure seamless transitions and continuity of care.
• Lead and participate in population health initiatives, such as ensuring timely health screenings for eligible patients.
• Assist with phone triage and follow-up outreach for patient support.
• Educate patients about their health conditions, care plans, and treatment options.
• Engage in promotional activities to enhance awareness and outreach for our services.
• Conduct home visits as necessary to provide care in a patient’s familiar environment.
• Report directly to the Medical Director, collaborating closely with operational leadership to enhance patient services.

Patient Population / Client Focus

Our primary focus is older adults on Medicare, with an emphasis on personalized primary care that maintains their health and well-being. By centering our services in the community, we work to keep patients actively engaged in their care journey, helping them to navigate their health challenges while enjoying a fulfilling life.

Qualifications

• A Master's degree in Nursing, a Doctor of Nursing Practice degree, or completion of an NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• An active and non-probationary state Nurse Practitioner license, along with an active DEA license.
• Clinical experience in primary care settings, specifically in internal medicine, geriatrics, or family medicine.
• Strong understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
• Ability to work collaboratively in a team-oriented environment, demonstrating excellent interpersonal skills.
• Passion for participating in an evidence-based approach to population health.
• Bilingual proficiency is preferred to better meet the diverse needs of our patient population.

Preferred Experience

• Experience in home care or community-based settings, specifically caring for the elderly population.
• Previous participation in quality improvement initiatives related to patient care.

Compensation and Benefits

We offer a competitive salary range of $120,000 to $160,000 annually, depending on experience and expertise. Additionally, our robust benefits package includes:
