Work Environment

This role is based in our outpatient office setting, where you will work alongside a collaborative care team consisting of physicians, other nurse practitioners, physician assistants, medical assistants, dedicated medical scribes, registered nurses, and care managers. Our facilities are designed to enhance patient care through seamless teamwork and communication, allowing healthcare providers to focus on what matters most: delivering exemplary primary care within a supportive environment.

Key Responsibilities

• Deliver exceptional primary care focusing on preventive measures and health promotion.
• Conduct office visits to address both routine health matters and acute concerns.
• Administer annual wellness visits and comprehensive health risk assessments, ensuring meticulous documentation and a holistic approach to care.
• Engage in care coordination with various providers, specialists, testing facilities, and external agencies to streamline patient care pathways.
• Lead population health initiatives in collaboration with the care team, ensuring that all eligible patients receive necessary screenings and preventative services.
• Assist the care team with phone triage, outreach efforts, and patient education regarding health conditions and treatment options.
• Participate in promotional events to enhance community awareness and engagement.
• Perform home visits when required to provide care and follow-up support.
• Report directly to the Medical Director, while working closely with operational leadership to enhance organizational effectiveness.

Patient Population / Client Focus

Advanced Cardiology PC is dedicated solely to serving older adults on Medicare. Our model is designed to provide personalized care that emphasizes maintaining patient independence and quality of life. Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an NP Certificate Program.
• Nationally certified as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• Active, non-probationary state Nurse Practitioner license.
• Current DEA license.
• Demonstrable experience in primary care settings such as internal medicine, geriatrics, or family medicine.
• Sound understanding of HCC documentation, ICD-10 coding practices, and conducting Health Risk Assessments (HRAs).
• Strong passion for teamwork and collaborative patient care.

Preferred Experience

• Experience working specifically with the Medicare population would be an advantage.
• Prior involvement in population health initiatives or quality improvement projects.
• Familiarity with electronic health records (EHR) systems and care coordination workflows.
