Advanced Cardiology PC, located in the vibrant Bronx, New York, is seeking dedicated and compassionate Full-Time Nurse Practitioners to join our team. This position will require you to deliver high-quality primary care services to our adult patients, particularly those enrolled in Medicare. Work Environment

This role is entirely onsite, allowing you to cultivate meaningful relationships with your patients in a collaborative, outpatient practice environment. Key Responsibilities

• Provide exceptional primary care, addressing routine and acute health issues.
• Conduct thorough Annual wellness visits and health risk assessments, focusing on holistic health and meticulous documentation.
• Coordinate care with various medical providers, specialists, testing facilities, and community resources to ensure comprehensive support for our patients.
• Lead population health initiatives, ensuring all eligible patients participate in evidence-based screenings, such as biennial breast cancer screenings.
• Assist the care team with phone triage and patient outreach, enhancing our communication and support structure.
• Educate patients about their health conditions, care plans, and treatment options, empowering them to take an active role in their health journey.
• Engage in community promotional activities to help raise awareness about our services and health initiatives.
• Conduct home visits as needed, providing personalized care in the comfort of patients’ homes.
• Report directly to the Medical Director, collaborating closely with operational leadership to drive quality care and organizational goals.

Patient Population / Client Focus

Our focus is primarily on older adults on Medicare, aiming to support their health and well-being through tailored primary care. Our approach is centered on maintaining a strong connection with our patients, fostering a relationship built on trust and open communication, which is essential for their ongoing health management and satisfaction.

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license.
• An active DEA license for prescribing controlled substances.
• Demonstrated experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• A solid understanding of Hierarchical Condition Categories (HCC) documentation, ICD-10 Coding, and Health Risk Assessments (HRAs).
• A strong commitment to teamwork and collaborative patient care, with excellent communication skills.

Preferred Experience

• Experience in leading population health initiatives and familiarity with managed care environments.
• Proficiency with electronic medical records (EMR) and other healthcare tech to streamline patient care delivery.
• Familiarity with concierge medicine practices and the unique healthcare needs of aging populations.

Compensation and Benefits

Advanced Cardiology PC offers a competitive salary ranging from USD 120,000 to 160,000 annually, commensurate with experience. Our comprehensive benefits package includes health insurance, retirement savings, and generous paid time off, promoting a healthy work-life balance.
