Work Environment

This role is entirely onsite, meaning you will be visiting patients in their homes throughout Yonkers. You'll work closely with physicians, nurses, and various allied health professionals to create a seamless continuum of care. Key Responsibilities

As a Nurse Practitioner at Gotham Enterprises, your responsibilities will include:

• Conducting thorough patient assessments, including history taking and physical examinations, to diagnose acute and chronic medical conditions.
• Developing, implementing, and adjusting individualized treatment plans based on patient needs, while efficiently ordering necessary diagnostic tests, such as bloodwork and imaging studies.
• Prescribing medications and therapies in accordance with state regulations and best practices, ensuring safe and effective pain management.
• Educating patients and their families about health conditions, preventive care, and wellness strategies to promote health literacy and improved outcomes.
• Collaborating with a multi-disciplinary team to coordinate comprehensive patient care, effectively communicating with physicians and specialists regarding patient progress and challenges.
• Maintaining accurate, detailed medical records that comply with HIPAA regulations and organization policies.
• Engaging in quality improvement initiatives aimed at enhancing care delivery and patient satisfaction across our organization.
• Remaining current on the latest healthcare practices, changes in regulations, and participating in continuing education to improve your skills and knowledge.

Patient Population / Client Focus

In this role, you will primarily serve patients within the home care setting, encompassing a diverse population that includes individuals recovering from surgery, managing chronic illnesses, elderly patients, and those requiring palliative care. This demographic often presents unique challenges and requires a compassionate approach tailored to individual needs and family dynamics.

Qualifications

The ideal candidate will possess the following qualifications:

• A current certification as a Nurse Practitioner.
• An active and unrestricted nursing license in New York State.
• A Master’s Degree in Nursing from an accredited institution.
• A minimum of 3 years of clinical experience, particularly in home care, primary care, or a similar relevant specialty.
• Exceptional communication and interpersonal skills, with the ability to foster rapport with patients and their families.
• Strong analytical and problem-solving capabilities to address complex patient needs effectively.

Preferred Experience

Additional preferred qualifications include experience working with care management systems and electronic health record (EHR) platforms. Compensation and Benefits
