Work Environment

Your working hours will typically be from 8:00 a.m. to 5:00 p.m., equating to a full-time commitment of 40 hours each week. Key Responsibilities

• Conduct comprehensive assessments to accurately diagnose both acute and chronic medical conditions affecting individuals of various ages.
• Develop individualized treatment plans that reflect the specific needs, preferences, and goals of each patient.
• Prescribe medications, therapies, and order necessary diagnostic tests, utilizing evidence-based practices to guide treatment.
• Educate patients on health maintenance, disease prevention, and effective lifestyle changes, empowering them to take active roles in their health journeys.
• Collaborate closely with interdisciplinary teams, including physicians, nurses, and healthcare specialists, to ensure seamless patient care and effective referrals.

Patient Population / Client Focus

Qualifications

• Active certification as a Nurse Practitioner.
• Current nursing license for the state of New York.
• Bachelor's degree in nursing from an accredited institution is required; a Master's degree is preferred.
• A minimum of 2 years of clinical experience as a Nurse Practitioner, ideally within primary care settings.
• Strong interpersonal and communication skills for effective patient engagement and education.
• Critical thinking and problem-solving abilities to address patient care challenges.

Preferred Experience

While a total of 2 years of experience is required, candidates with 3-5 years in primary care will be highly regarded. Familiarity with electronic health records systems and an understanding of school health initiatives are also advantageous for this role.
