Key Responsibilities

• Facilitate hospital admissions, transfers, and discharges, and coordinate appropriate inpatient and outpatient accommodations, ensuring smooth transitions of care.
• Coordinate patient care activities, including teaching sessions, counseling, discharge planning, and ensuring care delivery aligns with established priorities.
• Perform phone triage for acute clinical issues, conducting assessments following protocols and prescribing treatments as appropriate.
• Assist in the development, implementation, and evaluation of standards of care in collaboration with the healthcare team, adhering to institutional policies and regulatory requirements.
• Engage in ongoing clinical education and professional development to enhance skills and remain current in best practices and clinical guidelines.

Patient Population / Client Focus

The successful Nurse Practitioner will cater to a wide range of patients in a home care setting, including those experiencing behavioral health challenges. Qualifications

• Board-certified in Family Medicine or Internal Medicine.
• Active, unrestricted state license in New York.
• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• Authorization to work in the U.S.
• Excellent communication skills and the ability to create rapport with patients and families.
• A flexible and proactive approach to working in an onsite clinic environment.

Preferred Experience

• Prior experience in home care or behavioral health settings is highly desirable.
• Experience in handling acute clinical situations and familiarity with telehealth practices.

Compensation and Benefits

This position offers a competitive salary range of $150,000 to $160,000 per year, commensurate with experience and caseload management.
