Work Environment

Our services are delivered in a home care setting where Nurse Practitioners cater to the needs of patients in their own homes. This approach allows for personalized care and fosters a comfortable environment, particularly for those with chronic illnesses or mobility challenges. Employed on a full-time basis, Nurse Practitioners work 40 hours per week, from Monday to Friday, with a flexible schedule that supports work-life balance. We offer an onsite work setting designed to streamline collaboration with fellow team members and ensure optimal patient outcomes.

Key Responsibilities

As a Nurse Practitioner at Heart Care Center PC, your daily responsibilities will include:

• Conducting thorough physical examinations and reviewing patients' medical histories to develop effective care plans.
• Ordering and analyzing diagnostic tests, such as blood tests and imaging studies, to identify health issues accurately.
• Diagnosing medical conditions and formulating treatment plans for both chronic and acute illnesses.
• Prescribing medications in adherence to state regulations and ensuring that patients understand their treatment regimens.
• Maintaining meticulous patient records and schedules, contributing to seamless care transitions and continuity.
• Educating patients and their families about health conditions, treatment options, and preventative health measures.
• Fostering patient engagement through effective communication and by promoting healthy lifestyle choices.
• Collaborating closely with a multidisciplinary team, including physicians and other healthcare professionals, to ensure comprehensive patient care.
• Participating in ongoing training and research initiatives to enhance clinical expertise and contribute to the advancement of healthcare practices.

Patient Population / Client Focus

You will work with a diverse patient population, including individuals with chronic diseases, acute health issues, and those requiring ongoing health management. Your role will significantly impact the community’s health by delivering personalized care in the comfort of patients’ homes, allowing for better monitoring of conditions and fostering stronger relationships between patients and providers.

Qualifications

The ideal candidate will possess:

• A Master's Degree in Nursing with specialization as a Nurse Practitioner (ANP/FNP).
• A valid Nurse Practitioner license in New York State.
• An active DEA number, allowing for the prescription of medications.
• Proven clinical experience in areas such as primary care, cardiology, or urgent care.
• Strong knowledge of healthcare regulations and patient care best practices.

Preferred Experience

We prefer candidates with extensive experience in a clinical setting, particularly in management of chronic diseases and acute patient care. Bilingual candidates are encouraged to apply, as this can enhance communication with our diverse clientele.

Compensation and Benefits

Heart Care Center PC offers a competitive salary ranging from USD 140,000 to 180,000 annually, commensurate with experience. Our comprehensive benefits package includes:
