Key Responsibilities

• Conduct thorough assessments of patients presenting with cardiac symptoms or pre-existing conditions, documenting relevant medical histories.
• Order and interpret a range of diagnostic tests, including EKGs, stress tests, and echocardiograms, to inform clinical decisions.
• Develop personalized care plans that incorporate medication management, lifestyle changes, and patient education to empower individuals in managing their health.
• Collaborate actively with cardiologists, nurses, and other healthcare professionals to create comprehensive treatment strategies for patients.
• Monitor treatment effectiveness continuously, adjusting care plans as needed to ensure optimal results.
• Provide supportive counseling to patients and their families, helping them navigate the complexities of cardiac treatment options.
• Stay abreast of up-to-date advancements in cardiology, utilizing evidence-based practices to enhance patient care.

Patient Population / Client Focus

You will focus primarily on patients who reside in our assisted living facility and present with cardiac conditions or symptoms. This population may include elderly individuals with chronic heart disease, those recovering from heart surgery, and patients requiring ongoing monitoring for cardiovascular health. Your role will be pivotal in ensuring they receive timely and effective treatment, along with comprehensive education about their health conditions.

Qualifications

• Must possess a valid, unencumbered Nurse Practitioner license, free from any open or unresolved disciplinary actions.
• Current national certification from ANCC or AANP in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care, or the ability to obtain certification by your start date.
• Physical capability to perform job duties, including the ability to lift a 30-pound bag in and out of a vehicle, navigate stairs, and conduct physical assessments that may require sitting, standing, or kneeling.

Preferred Experience

A minimum of two years of relevant clinical experience in cardiology, gerontology, internal medicine, or endocrinology is preferred. Understanding the complexities associated with managing heart conditions in a geriatric population will be advantageous for this role.

Compensation and Benefits
