Work Environment

This position is based onsite at our Assisted Living Facility in the Bronx, where a collaborative and supportive atmosphere prevails. Key Responsibilities

• Conduct thorough health histories and perform comprehensive physical examinations to establish accurate diagnoses.
• Formulate and manage individualized treatment plans utilizing pharmacological and non-pharmacological interventions for pain management.
• Provide education and counseling to patients about their pain management therapies, emphasizing the importance of compliance and potential medication side effects.
• Assess and provide ongoing care for patients dealing with chronic pain issues, continuously adjusting treatment plans as necessary.
• Prescribe medications in accordance with established treatment protocols and maintain patient safety by checking the Prescription Monitoring Program (PMP).
• Collaborate with physicians to determine optimal treatment strategies tailored to each patient's unique needs.
• Utilize strong clinical assessment skills to evaluate complex pain disorders effectively.
• Develop and implement comprehensive pain management plans based on the latest research and best practices.
• Exhibit excellent communication and interpersonal skills to foster strong relationships with patients and team members alike.

Patient Population / Client Focus

The Nurse Practitioner will serve a diverse population of patients primarily within an assisted living context. This may include elderly residents who are experiencing chronic pain resulting from conditions like arthritis, neuropathy, and other age-related disorders. Qualifications

• Master's Degree in Nursing (MSN) from an accredited institution.
• Current and valid New York State Nurse Practitioner license.
• A minimum of 2 years of clinical experience in pain management or a related field.

• Proven ability to assess, diagnose, and treat complex pain-related issues.
• Experience in pharmacological management of pain, including appropriate prescribing practices.
• Proficiency in performing modalities such as nerve blocks, spinal injections, and pain pump management.

Preferred Experience

• Experience working within an Assisted Living Facility or similar environment.
