Key Responsibilities

• Conduct comprehensive evaluations and cardiac examinations on patients presenting with cardiovascular concerns, utilizing advanced diagnostic techniques.
• Diagnose and formulate management plans for a variety of cardiovascular conditions such as coronary artery disease, heart failure, arrhythmias, and hypertension.
• Collaborate closely with our interventional cardiologists to create individualized treatment plans tailored to each patient’s specific conditions and lifestyle needs.
• Order, perform, and interpret critical diagnostic tests including EKGs, Holter monitors, and stress tests to assist in the accuracy of diagnoses and treatment plans.
• Implement detailed treatment protocols that incorporate medication management, lifestyle alterations, patient education, and follow-up strategies.
• Coordinate with a multidisciplinary team, including cardiologists, nurses, dietitians, and other healthcare professionals, ensuring consistent communication and optimal patient outcomes.
• Develop rapport with patients and their families, providing thorough education on health management and self-care strategies essential for living with cardiovascular diseases.

Patient Population / Client Focus

In this role, you will serve a diverse patient population in the Bronx, encompassing individuals with varying degrees of cardiovascular health challenges. Qualifications

• A Master's degree in Nursing from an accredited Nurse Practitioner program.
• Active and unencumbered Registered Nurse (RN) License from the State of New York.
• Certification in Basic Life Support (BLS) is required.
• Proficiency in electronic medical records (EMR) systems for efficient patient documentation and data management.
• Strong communication, leadership, and interpersonal skills, enabling effective collaboration in a fast-paced environment.

Preferred Experience
