We are seeking a committed and experienced Adult-Gerontology Nurse Practitioner to join our team at Center Point Healthcare Solution Management Practice in the Bronx, New York. Work Environment

This position will be based onsite in a school setting within the Bronx, an area known for its diverse community and rich cultural backdrop. As part of a multidisciplinary team, you will work alongside physicians, nurse practitioners, medical assistants, and care managers. Our office operates during standard hours from Monday to Friday, 9:00 AM to 5:00 PM, creating a consistent and manageable work schedule for our providers.

Key Responsibilities

• Provide exceptional primary care to a diverse patient population, addressing both routine and acute health issues.
• Conduct thorough office visits that include annual wellness assessments, health risk evaluations, and developing individualized care plans based on detailed patient history and needs.
• Collaborate effectively with a robust healthcare team to ensure seamless care coordination among specialists, testing facilities, and therapeutic agencies.
• Lead initiatives in population health to ensure adherence to evidence-based guidelines, such as facilitating breast cancer screenings for eligible female patients.
• Engage in phone triage and outreach to enhance patient communication and improve follow-up processes.
• Educate patients and their families about health conditions, treatment options, and lifestyle modifications to foster a deep understanding of their care.
• Participate in community outreach and promotional activities to enhance awareness about our services and drive engagement.
• Perform home visits as necessary to support patients who may have difficulty accessing traditional care settings.
• Contribute to organizational goals by reporting directly to the Medical Director and collaborating with operational leadership.
• Perform additional duties as assigned to support the overall function of the healthcare practice.

Patient Population / Client Focus

This role primarily focuses on older adults on Medicare, utilizing a personalized approach to primary care that is designed to keep patients healthy and engaged in their health management. You will be pivotal in developing long-term relationships with patients, fostering trust, and ensuring they feel connected to their care plan. By engaging holistically with our community, our practitioners aim to emphasize quality over quantity in healthcare delivery.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of a Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the AANP or ANCC.
• An active, non-probationary Nurse Practitioner license in the state of New York.
• Current DEA license to prescribe medications.
• A minimum of 2 years of experience in a primary care setting, such as internal medicine, geriatrics, or family medicine.
• Proficient understanding of HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) coding, and health risk assessments (HRAs).
• A team-oriented mindset with a passion for cross-functional collaboration and shared patient outcomes.

Preferred Experience

• Experience working with underserved populations in urban settings.
• Familiarity with electronic health records (EHR) systems and telehealth platforms.
• Demonstrated leadership capabilities within a healthcare setting.
• Proven ability to educate patients effectively about complex medical conditions and treatment plans.

Compensation and Benefits

We offer a competitive salary range from USD 120,000 to 160,000 annually, commensurate with experience and qualifications.
