This role is situated within an Assisted Living Facility that caters to a diverse group of seniors, many of whom have complex medical needs. Key Responsibilities

• Function autonomously to assess, diagnose, and manage patients with acute, chronic, critical, and complex health conditions.
• Develop and implement individualized treatment plans based on patient assessments and collaborate with interdisciplinary teams to optimize care delivery.
• Prescribe and manage medications, including determining appropriate frequency and dosage, while ensuring compliance with legal and ethical standards.
• Record and maintain comprehensive patient records in a timely manner, ensuring that all documentation is accurate and accessible to relevant healthcare providers.
• Facilitate communication and patient transitions between various levels of care, ensuring seamless integration within the healthcare system.
• Engage in proactive health management discussions with patients and families, providing education and support for lifestyle modifications and adherence to treatment plans.

Patient Population / Client Focus

You will primarily work with the elderly population residing in our Assisted Living Facility, many of whom present with multifaceted health challenges such as chronic illnesses, mobility issues, and cognitive impairments. Your bilingual skills will be invaluable in ensuring that all residents receive equitable care, respect, and attention to cultural competence.

Qualifications

• Current Nurse Practitioner certification and an active New York state nursing license.
• A Master’s degree in nursing from an accredited program.
• A minimum of 3 years of clinical experience, preferably within geriatric or long-term care settings.
• Proficient in both Spanish and English, with exceptional communication and interpersonal abilities.
• Demonstrated strong analytical, critical thinking, and problem-solving skills.

Preferred Experience

• Experience in an Assisted Living or long-term care facility is highly desirable.
• Background in managing chronic diseases and coordinating care for elderly patients.
• Familiarity with electronic medical record (EMR) systems and telehealth technologies.

Compensation and Benefits

The position offers a competitive salary range of $120,000 to $130,000 annually, commensurate with experience and qualifications.
