Position Overview

Parkland Health and Hospital PC, located in West Hempstead, New York, is seeking a dedicated and compassionate Nurse Practitioner to join our dynamic team in providing exceptional primary care services. This position is full-time, with a commitment of 40 hours per week, Monday through Friday, ending at 5:00 PM. The ideal candidate will play a crucial role in diagnosing, treating, and advocating for patients, ensuring they achieve optimal health outcomes.

Key Responsibilities

• Conduct thorough physical examinations and assess patient medical histories to develop comprehensive care plans.
• Order, interpret, and analyze diagnostic tests, including blood tests and X-rays, to accurately diagnose health conditions.
• Diagnose acute and chronic health issues and create effective treatment plans for a diverse range of medical concerns.
• Prescribe medications and provide necessary education regarding treatment options, side effects, and management strategies.
• Maintain meticulous records of patient encounters, treatment plans, and follow-up appointments.
• Communicate effectively with patients about their diagnoses, treatment plans, and preventive measures.
• Facilitate health education sessions to inform patients and their families about diseases, preventive measures, and healthy lifestyle choices.
• Collaborate with other healthcare providers to ensure continuity of care and support patients' journey to wellness.
• Continuously enhance knowledge of clinical best practices and healthcare innovations through participation in professional development and research initiatives.
• Promote community health initiatives that align with the organization’s goals to improve the overall health outcomes of our patient population.

Patient Population / Client Focus

As a Nurse Practitioner at Parkland Health, you will work with a diverse patient population, encompassing various ages and backgrounds. Your primary focus will be on promoting health maintenance, disease prevention, and managing chronic health conditions such as diabetes, hypertension, and cardiovascular diseases. You will be pivotal in guiding patients through their healthcare journeys and helping them make lifestyle changes aimed at improving their overall wellbeing.

Qualifications

• A Master’s degree in Nurse Practitioner studies.
• A valid New York State Nurse Practitioner license, with ANP or FNP certification.
• Active DEA number is required.
• Proven experience as a Nurse Practitioner, preferably in primary care or a similar setting.
• Strong ethics and a commitment to providing high-quality patient care.
• Excellent communication skills and a positive attitude that fosters teamwork and collaboration.
• Patience, problem-solving skills, and a genuine desire to enhance patient experiences.
• Willingness to stay updated with medical advancements and participate in ongoing education.

Preferred Experience

• Experience in areas such as acute care, internal medicine, cardiology, family health, or urgent care.
• Bilingual capabilities are a plus but not a requirement.
• Extensive clinical experience in primary care settings is highly valued.

Compensation and Benefits
