Work Environment

This position is based onsite at a school, providing you with a unique setting where you can directly impact the health of children and families within the educational environment. Your work hours will generally span from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m., and participation in weekend community outreach events, providing a balanced work schedule of approximately 40 hours per week.

Key Responsibilities

• Conduct thorough health assessments and screenings for students and their families, addressing acute and chronic medical conditions.
• Develop personalized care plans that include preventive care strategies and health promotion initiatives.
• Diagnose and manage a variety of conditions while ensuring culturally sensitive care tailored to the unique needs of our diverse population.
• Collaborate with local organizations to design and implement effective health education programs, fostering a culture of wellness.
• Coordinate referrals to specialists and facilitate follow-up care to ensure continuity of care for all patients.
• Maintain accurate medical records in compliance with regulatory standards, utilizing electronic health records (EHR) systems efficiently.
• Engage actively in community outreach events and health fairs to increase awareness and provide essential health screenings.
• Stay informed about emerging community health trends and best practices to ensure we are providing top-notch care.

Patient Population / Client Focus

In this role, you will primarily serve students and their families, promoting health equity and targeted care for various populations. Your efforts will be crucial in addressing health disparities, empowering families with knowledge and resources to improve their overall well-being.

Qualifications

• Master's or Doctoral degree in Nursing from an accredited program.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Current, unrestricted Nurse Practitioner (APRN) license in New York State.
• Valid DEA and NPI numbers are required.
• Strong clinical assessment and diagnostic skills, coupled with excellent communication capabilities.
• Proficiency in electronic health records (EHR) systems and the ability to work independently as well as part of a collaborative team.

Preferred Experience

• A minimum of 3-5 years of clinical experience, particularly in primary care, community health, or public health settings.
• Experience engaging diverse populations and understanding their unique health needs.

Compensation and Benefits
