Work Environment

This role is based onsite and requires a compassionate approach to care delivery. As a member of our team, you will work under a dynamic schedule typically from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. The collaborative environment includes participation in community events, health fairs, and outreach activities, emphasizing our commitment to the local population's health and well-being.

Key Responsibilities

• Conduct thorough health assessments and health screenings tailored to the needs of individuals and families within the community.
• Diagnose and manage both acute and chronic medical conditions, offering preventive care strategies and health promotion activities.
• Engage with community organizations and stakeholders to create and implement relevant health education initiatives that inspire healthy living.
• Deliver culturally sensitive care, advocating for patients' rights and ensuring access to a broad range of healthcare services.
• Coordinate referrals and provide follow-up care to guarantee sustained health outcomes for patients.
• Maintain meticulous and compliant medical records, ensuring they meet all regulatory standards and protocols.
• Participate in community outreach and health screenings during weekends, solidifying our presence in the community.
• Stay abreast of current trends in community health, evidence-based practices, and healthcare policies relevant to our patient population.

Patient Population / Client Focus

This position focuses on a diverse patient population within the community, including underserved and vulnerable individuals who may face barriers to healthcare access. You will be responsible for understanding and addressing the unique cultural, socioeconomic, and health challenges of these populations, thereby making a significant impact on their health outcomes.

Qualifications

• Currently certified as a Nurse Practitioner with an active state nursing license in New York.
• A Master's degree in nursing from an accredited institution is required.
• A minimum of three years of clinical experience in community health, public health settings, or relevant specialized areas is essential.
• Outstanding interpersonal and communication skills, enabling effective collaboration with diverse patient groups.
• A strong commitment to social justice, health equity, and advocacy for vulnerable populations in the community.

Preferred Experience

Compensation and Benefits

We offer a competitive salary range of USD 180,000 – 200,000 per year, commensurate with your experience and qualifications. In addition to a generous salary, we provide a comprehensive benefits package that includes:
