Position Overview

The Heart Care Center PC is actively seeking dedicated and skilled Nurse Practitioners to join our dynamic team in Brooklyn, New York. As an integral part of our primary care practice, you will have the opportunity to diagnose and treat various health conditions while emphasizing preventative care and healthy lifestyle promotion. This full-time role requires a commitment to optimal patient care from Monday to Friday, with a standard work schedule of 40 hours each week, concluding at 5:00 PM.

Work Environment

You will work in an onsite assisted living facility, which allows for direct interaction with residents in a supportive, community-oriented setting. Key Responsibilities

As a Nurse Practitioner at Heart Care Center PC, your responsibilities will include:

• Conducting thorough physical examinations and documenting patient medical histories.
• Ordering and interpreting diagnostic tests such as blood work and imaging studies.
• Diagnosing health conditions and creating tailored treatment plans.
• Prescribing medications and providing detailed information to patients regarding their treatment.
• Maintaining accurate and up-to-date patient records and schedules.
• Educating patients and their families on health conditions, disease prevention, and healthy practices.
• Overseeing the overall health management of your patients.
• Participating in team meetings and contributing to ongoing quality improvement initiatives.
• Conducting research and staying current with medical advancements to enhance professional knowledge.

Patient Population / Client Focus

Your focus will be on a diverse population of residents in the assisted living facility, many of whom may have chronic health conditions requiring ongoing management. You will play a critical role in ensuring patients receive individualized care tailored to their unique health challenges, promoting not only their physical well-being but their mental and emotional health as well.

Qualifications

The ideal candidate will possess the following qualifications:

• A valid Nurse Practitioner license in New York State.
• A Master’s Degree in Nursing with a specialization as a Nurse Practitioner, either in Adult Nurse Practitioner (ANP) or Family Nurse Practitioner (FNP).
• A DEA number to prescribe medications.
• Demonstrated expertise in primary care, acute care, and chronic disease management.
• Strong understanding of healthcare regulations and best practices.
• Excellent communication, interpersonal, and organizational skills.
• A commitment to ethical practice and a collaborative team-oriented approach.

Preferred Experience

We highly value candidates with proven experience in healthcare settings such as clinics, hospitals, or urgent care facilities. Therefore, experience in specialties like internal medicine, cardiology, or emergency medicine is preferred. Bilingual candidates are a plus as they can communicate effectively with a diverse patient population.

Compensation and Benefits

Heart Care Center PC offers a competitive salary range of USD 140,000 to 180,000 annually, commensurate with experience. Additionally, we provide a comprehensive benefits package, including:
