Work Environment

This full-time role is based onsite at one of our hospitals, fostering a collaborative and engaging environment. The Nurse Practitioner will work alongside medical assistants, dedicated scribes, nurses, and care managers to facilitate a team-based approach to healthcare. Our outpatient practices emphasize continuity of care, allowing practitioners to establish lasting relationships with patients and enhance community health outcomes.

Key Responsibilities

• Deliver exceptional primary care to patients through comprehensive assessments and individualized treatment plans.
• Conduct both routine and acute office visits, ensuring thorough documentation and follow-up as necessary.
• Administer Annual wellness visits and health risk assessments, focusing on holistic patient health and accurate documentation.
• Facilitate care coordination amongst healthcare providers, specialists, and testing facilities, ensuring seamless patient experiences.
• Lead initiatives in population health, guaranteeing that patients receive timely screenings and preventive care.
• Assist the care team with phone triage and patient outreach, strengthening communication and care continuity.
• Educate patients thoroughly on their health conditions, treatment options, and necessary lifestyle modifications.
• Participate in promotional activities, enhancing community engagement and awareness of services.
• Conduct home visits for patients who require additional support or follow-up care.
• Other responsibilities as assigned by operational leadership, contributing to the overall performance of the care team.

Patient Population / Client Focus

In this role, you will primarily serve a diverse elderly population enrolled in Medicare. Our care model emphasizes personalized primary care, allowing our Nurse Practitioners to address the unique health challenges faced by older adults. With a focus on maintaining quality of life, you will support patients in managing chronic conditions, preventive care, and lifestyle improvement.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of a Nurse Practitioner Certificate Program.
• Current national certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner, recognized by AANP or ANCC.
• Active, non-probationary Nurse Practitioner license in New York.
• Valid DEA license for prescribing medications.
• Proven experience in primary care settings, focusing on internal medicine, geriatrics, or family medicine.
• Comprehensive understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments.

Preferred Experience

• Previous outpatient primary care experience is highly desirable.
• Familiarity with value-based care models and advanced care coordination strategies.
• Experience using electronic health record systems for documentation and patient management.

Compensation and Benefits

The annual salary range for this position is USD 120,000 – 160,000, commensurate with experience and qualifications.
