Work Environment

This position is performed onsite in a home care facility that promotes a collaborative and supportive atmosphere among healthcare professionals. You will work as part of a dynamic care team that includes a physician, nurse practitioners, physician assistants, medical assistants, and care managers, all dedicated to the continuity of patient care. Our collaborative approach emphasizes teamwork and allows for comprehensive patient management within the clinic setting.

Key Responsibilities

• Deliver exceptional primary care services to patients, often conducting both routine and acute office visits.
• Manage annual wellness visits and health risk assessments, ensuring thorough documentation and a holistic view of health.
• Coordinate care with other healthcare providers, specialists, and agencies to enhance patient outcomes and streamline care delivery.
• Exercise leadership in population health initiatives, ensuring adherence to evidence-based practices such as regular breast cancer screenings for eligible patients.
• Participate in patient outreach and phone triage to provide timely support and follow-up for ongoing health concerns.
• Educate patients about their health conditions, treatments, and care plans, ensuring they understand and feel empowered in their healthcare journeys.
• Engage in promotional activities to strengthen community relationships and support clinic initiatives.
• Conduct home visits when necessary, providing personalized care in the comfort of the patient's environment.
• Perform additional duties as assigned by the Medical Director, and work closely with operational leadership to enhance clinic operations.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or a graduate from an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• Active Nurse Practitioner license in New York state that is non-probationary.
• Current DEA license for addressing pain management and medication prescriptions.
• Experience in primary care, particularly in the realms of internal medicine, geriatrics, or family medicine.
• Knowledge and understanding of HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
• Strong collaborative spirit and a desire to work in a team-oriented environment.

Preferred Experience

• Prior experience in outpatient settings, particularly in clinics focused on elderly care.
• Demonstrated capability in leading patient education initiatives.
• Familiarity with electronic health record systems for efficient patient documentation and care coordination.
• A track record of engaging patients in their care plans effectively.

Compensation and Benefits

This competitive position offers an annual salary ranging from USD 120,000 to USD 160,000, commensurate with experience and qualifications.
