Position Overview

Key Responsibilities

• Conduct thorough health assessments, including comprehensive physical exams and screenings, tailored to our diverse resident population.
• Diagnose and manage both acute and chronic medical conditions, prioritizing preventive care measures to enhance health outcomes.
• Develop and implement personalized treatment plans in partnership with patients and members of our multidisciplinary teams, ensuring a holistic approach to health care.
• Provide culturally competent care, advocating for patients' rights and ensuring they have access to essential healthcare services.
• Facilitate referrals and coordinate follow-up care to maintain continuity of care, ensuring that all aspects of each patient’s health are considered.

Qualifications

• A Master’s degree in Nursing from an accredited institution.
• Current certification as a Nurse Practitioner with an active state nursing license.

Compensation and Benefits

• Attractive salary ranging from $150,000 to $160,000 per year based on experience and qualifications.
