Full-Time Nurse Practitioner - Assisted Living Facility Care

Position Overview

Advanced Cardiology PC is actively seeking dedicated and compassionate Nurse Practitioners to join our dynamic team in Massapequa, New York. This full-time role is designed for professionals who are committed to providing exceptional primary care to the Medicare population residing in assisted living facilities. The Nurse Practitioner will be instrumental in delivering longitudinal care while fostering meaningful patient relationships that promote health and well-being.

This role is situated in an assisted living facility where you will have the opportunity to work alongside a collaborative care team that includes physicians, fellow Nurse Practitioners, medical assistants, and care managers. Key Responsibilities

• Deliver comprehensive primary care services to residents, ensuring high-quality clinical outcomes.
• Conduct thorough office visits for both routine check-ups and acute medical issues, utilizing evidence-based practices.
• Administer annual wellness visits and health risk assessments, ensuring meticulous documentation that reflects the holistic view of patient health.
• Facilitate care coordination among a broad spectrum of healthcare providers, specialists, testing facilities, and external agencies to optimize patient care.
• Lead population health initiatives by educating patients about screenings and preventive services, such as annual breast cancer screenings for eligible patients.
• Support the care team with telephonic triage and outreach, addressing patients' needs proactively.
• Engage patients in health education related to their conditions, treatment plans, and lifestyle modifications.
• Participate in community outreach and promotional activities that align with our mission to enhance community health.
• Perform home visits as necessary, providing compassionate and personalized care in the patients' living environments.
• Collaborate with the Medical Director and operational leadership to ensure alignment with organizational goals and strategies.

Patient Population / Client Focus

Your primary focus will be on older adults residing in assisted living facilities, primarily those enrolled in Medicare. This unique patient population often faces complex health challenges that require thoughtful and individualized approaches. You will play a pivotal role in enhancing their quality of life by addressing their physical, emotional, and social health needs.

Qualifications

• Must possess a Master of Science in Nursing, Doctor of Nursing Practice, or be a graduate of an accredited Nurse Practitioner Certificate Program.
• National Certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner from the AANP or ANCC is required.
• Possession of an active and non-probationary state Nurse Practitioner license.
• Active DEA license holder to prescribe necessary medications.
• Experience in primary care settings, specifically in internal medicine, geriatrics, or family medicine.
• Proficient knowledge of documentation practices related to HCC (Hierarchical Condition Categories), ICD-10 coding, and health risk assessments.
• Strong team-oriented mindset, with excellent communication skills and a commitment to collaborative patient care.

Preferred Experience

• Previous experience working in assisted living or similar facility settings.
• Demonstrated ability to develop and implement care plans that meet diverse patient needs.
• Experience in population health management or community health initiatives.

Compensation and Benefits

We offer a competitive salary ranging from USD 120,000 to 160,000 per year, commensurate with experience and qualifications. Our comprehensive benefits package includes health, dental, and vision insurance, retirement plans, and continuing education reimbursement.
