Key Responsibilities

• Conduct thorough assessments by analyzing and interpreting patients’ medical histories, symptoms, and physical findings to formulate accurate diagnoses.
• Design evidence-based treatment plans utilizing current scientific rationale and adhering to established standards and guidelines to ensure optimal patient outcomes.
• Diagnose and manage acute and chronic health issues, including illnesses, infections, and injuries, providing timely interventions.
• Prescribe medications judiciously, taking into account efficacy, safety, and cost-effectiveness in alignment with legal and professional standards.
• Implement behavioral modification strategies tailored to individual patient needs, aiming to improve health risk factors.
• Provide comprehensive education for patients regarding self-management of both acute and chronic illnesses, adjusting instructions based on their unique circumstances.
• Utilize the Electronic Health Record (EHR) system efficiently to document patient interactions and treatment plans, ensuring compliance with organizational policies.
• Order, perform, or interpret diagnostic tests including CBCs, EKGs, and radiographs to aid in patient assessments.

Qualifications

• Must possess a current Nurse Practitioner certification issued by the New York State Education Department (NYSED).
• A valid New York State license and current registration to practice as a Registered Professional Nurse must be maintained.
• Basic Life Support (BLS) certification from the American Heart Association (AHA) is required, either existing or obtained during orientation.

Preferred Experience

While not mandatory, candidates with previous experience in assisted living or geriatric care are highly preferred, as this experience will enrich your ability to address the unique healthcare needs of our residents. Compensation and Benefits
