Work Environment

Your work will primarily take place onsite at our Assisted Living Facility in Franklin Square, which serves a diverse population in a supportive environment. The facility is designed to offer a calming atmosphere conducive to healing and well-being. As part of a multidisciplinary team, you will collaborate closely with mental health professionals, social workers, and support staff, ensuring a cohesive approach to patient care. The typical work schedule is from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. You may also participate in community outreach events and health fairs on weekends.

Key Responsibilities

• Conduct thorough health assessments and screenings to determine the physical and mental health needs of residents, ensuring a holistic treatment approach.
• Diagnose a range of mental health conditions, managing both acute and chronic issues through evidence-based treatment modalities.
• Develop and implement care plans in collaboration with residents, families, and other healthcare professionals, promoting shared decision-making.
• Engage in community education initiatives and health programs aimed at raising awareness about mental health and creating pathways for access to care.
• Advocate for patients within the healthcare system, ensuring their rights and access to necessary health services are prioritized.

Patient Population / Client Focus

Qualifications

• Current certification as a Nurse Practitioner with an active New York state nursing license.
• A Master’s degree in Nursing from a recognized and accredited program.

Compensation and Benefits

We offer a competitive salary ranging from $180,000 to $200,000 annually, based on your experience and qualifications.
