Work Environment

This role is primarily onsite at our assisted living facility in Hempstead, New York, which serves a diverse population of residents with varying healthcare needs. Key Responsibilities

• Conduct thorough physical examinations of residents to evaluate health status and identify potential health issues.
• Diagnose and manage both acute and chronic medical conditions, developing tailored treatment plans based on individual patient needs.
• Order and interpret diagnostic tests, such as laboratory work and medical imaging, to support accurate assessments.
• Administer prescribed medications and treatments while adhering to safety protocols and ensuring residents' comfort and well-being.
• Keep detailed and precise medical records using ICD-9 coding systems, ensuring compliance with healthcare regulations.
• Collaborate with a team of healthcare professionals to ensure effective communication and continuity of care for all residents.
• Educate patients and their families on health maintenance, disease prevention, and management strategies to empower them in their health journey.

Patient Population / Client Focus

This role will primarily focus on the geriatric population residing in the assisted living facility, addressing specific health concerns commonly found in older adults. You will encounter a broad spectrum of conditions, including but not limited to cardiovascular issues, diabetes, and cognitive impairments, with a strong emphasis on promoting overall wellness and quality of life.

Qualifications

• A Bachelor’s Degree in Nursing from an accredited institution.
• A minimum of two years of clinical experience, preferably in urgent care, geriatrics, or a related specialty.
• Certification as a Nurse Practitioner and unrestricted licensure to practice in New York State.

Preferred Experience

• Experience with electronic health records systems, such as Cerner, for comprehensive patient documentation and care coordination.
• Familiarity with procedures, including botulinum toxin treatments, and knowledge of geriatric care practices.
• Previous involvement in interdisciplinary teams, highlighting collaboration and communication skills in a healthcare setting.

Compensation and Benefits
