Work Environment

Our Assisted Living Facility provides a vibrant and supportive environment where healthcare professionals can excel. You will work directly with a dynamic team comprised of physicians, nurses, social workers, and rehabilitation specialists. The clinic operates onsite, allowing for immediate access to resources and interdepartmental collaboration. Your typical working hours will be from 9:00 AM to 6:00 PM with occasional evening clinics extending to 8:00 PM to accommodate our residents' needs.

Key Responsibilities

• Conduct thorough health assessments and screenings of residents and their families, ensuring each individual's unique healthcare needs are addressed.
• Diagnose and manage both acute and chronic medical conditions, guiding residents in preventive care and health promotion activities.
• Design and implement tailored health education programs in collaboration with community organizations to enhance public health awareness.
• Advocate for patients by providing culturally sensitive care and addressing their rights and access to necessary health services.
• Ensure continuity of care by coordinating patient referrals and following up on treatment plans and outcomes.
• Maintain high standards in documentation practices by keeping accurate and detailed medical records in compliance with health regulations.
• Engage in community outreach initiatives and health fairs, providing valuable health screenings and information to underserved populations.
• Stay informed about the latest community health trends, evidence-based practices, and necessary policy changes to continually enhance care delivery.

Patient Population / Client Focus

In this role, you will primarily focus on the residents of our Assisted Living Facility. This population often consists of elderly individuals with varying healthcare needs, including management of chronic diseases, mobility challenges, and preventive care. As a Nurse Practitioner, you will provide comprehensive care with a strong emphasis on dignity, respect, and patient advocacy, guiding residents toward improved health outcomes.

Qualifications

• A Master’s degree in Nursing from an accredited program.
• Current certification as a Nurse Practitioner, with the ability to practice in New York.
• Minimum of 3 years of clinical experience in a relevant healthcare setting, preferably with older adults.
• Exceptional interpersonal and communication skills, including the ability to connect and work effectively with diverse populations.
• A dedication to social justice, health equity, and advocacy for vulnerable communities.

Preferred Experience

• Experience in geriatric care or within an assisted living environment is highly advantageous.

Compensation and Benefits

Vital Key Health Clinic offers a competitive salary range of USD 150,000 – 160,000 per year, commensurate with experience and qualifications.
