The work setting is primarily onsite at our well-established assisted living facility in Freeport, New York. Key Responsibilities

As a Nurse Practitioner at Support Individuals Healthcare Services, you will:

• Conduct Thorough Assessments: Perform comprehensive physical examinations, health assessments, and history-taking for residents to determine their medical needs and challenges.
• Diagnose and Treat Patients: Diagnose and manage acute, episodic, or chronic illnesses and prescribe appropriate medications and treatments.
• Manage Patient Care: Develop and implement individualized patient management plans that address each resident's unique health conditions, ensuring continuity of care.
• Collaborate with Healthcare Team: Communicate with collaborating physicians and specialists to coordinate patient care effectively, ensuring the seamless transfer of information and referrals.
• Document Patient Progress: Maintain accurate and up-to-date medical records using electronic health record systems while minimizing paperwork to focus on patient care.
• Educate Residents and Families: Provide education and counseling to residents and their families regarding health conditions, treatment options, and preventive measures.
• Lead Initiatives: Actively participate in the development and implementation of patient management policies and procedures, contributing to the continuous improvement of our care delivery.
• Foster Relationships: Build rapport with residents to understand their needs better and create a supportive healthcare experience.
• Participate in Community Engagement: Engage with low-income populations and participate in health outreach initiatives to promote wellness and preventive care.

Patient Population / Client Focus

This role primarily serves a diverse resident population within our assisted living facility, with a concentration on seniors and low-income individuals. Understanding the unique healthcare challenges faced by these communities is crucial, as our care strategies are tailored to meet their specific needs. You will play an essential role in ensuring that our residents receive dignified, respectful, and culturally competent care.

Qualifications

To be considered for this role, candidates must possess the following qualifications:

• A Master's Degree in Nursing or a Doctor of Nursing Practice (DNP) or completion of an accredited NP program.
• Current national certification from a recognized certifying body, such as AANP or ANCC.
• Active and unrestricted state license to practice as a Nurse Practitioner.
• Active DEA license for prescribing medications.
• Authorized to work in the United States.

Preferred Experience

We value candidates with prior experience working in assisted living settings, geriatrics, or with vulnerable populations. Strong leadership skills, excellent verbal and written communication abilities, and a passion for enhancing healthcare access for underserved communities are highly desirable. Proficiency in Microsoft Office and electronic health record systems will be considered an asset.

Compensation and Benefits
