Work Environment

As part of our concierge medicine model, your work will primarily take place onsite during visits to patients' homes, enabling you to deliver personalized care in a comfortable environment. Key Responsibilities

• Conduct thorough assessments and effectively diagnose both acute and chronic medical conditions through direct patient interaction in their homes.
• Create, implement, and evaluate personalized treatment plans, and proficiently order and interpret necessary diagnostic tests to guide your decision-making.
• Prescribe medications and therapies based on evidence-based practices while closely monitoring patient responses to treatment.
• Deliver health education to patients and their families on topics such as disease prevention, lifestyle modifications, and health promotion strategies to foster better health outcomes.
• Coordinate patient care by actively collaborating with physicians, nurses, and specialists to ensure a seamless healthcare experience.
• Maintain accurate and comprehensive medical records in our electronic health record (EHR) system to ensure continuity of care and compliance with regulatory standards.
• Participate in quality improvement initiatives aimed at enhancing patient care and operational efficiency, as well as complying with healthcare regulations and best practices.
• Stay abreast of advancements in clinical practice and evolving healthcare policies to provide the most current knowledge and care methods to our patients.

Patient Population / Client Focus

You will be caring for a diverse patient population, ranging from young families to seniors, who require personalized healthcare solutions. The flexibility of our house call service allows for in-depth discussions in a familiar setting, facilitating better understanding and adherence to treatment plans.

Qualifications

• Current certification as a Nurse Practitioner is required.
• An active state nursing license to practice in New York.
• A Bachelor’s degree in Nursing followed by a Master’s degree in Nursing from an accredited program.
• Minimum of 3-5 years of clinical experience in relevant specialties, with a strong focus on family medicine.
• Exceptional communication and interpersonal skills to establish rapport with patients and their families.
• Demonstrated analytical and problem-solving abilities that enhance patient assessment and care.

Preferred Experience

• Experience in concierge medicine or similar models that emphasize patient-focused care.

Compensation and Benefits

We offer a highly competitive salary range of USD 150,000 – 160,000 annually, commensurate with experience and qualifications. Our comprehensive benefits package includes:
