Evergreen Family Practice is actively seeking a dedicated and experienced Family Nurse Practitioner to provide excellent patient-centered care as part of our concierge medicine team. This full-time role is based onsite in West Hempstead, New York, where you will use your nursing expertise to promote health and wellness in a supportive and collaborative environment. You will be leading a team that includes X-Ray Technologists, Nurses, and Front Office Specialists in a setting designed to uplift both staff and patients.

Key Responsibilities

• Lead and mentor clinic staff in delivering exceptional patient care while modeling exemplary behaviors, attitudes, and professionalism.
• Ensure the accurate and timely completion of patient charts and effectively manage information flow for optimal patient outcomes.
• Collaborate closely with the Supervising Physicians to implement clinical protocols and maintain high standards of care.
• Order, perform, and interpret diagnostic tests, responsibly prescribe treatments, and educate patients on managing their health conditions.
• Conduct therapeutic procedures such as incision and drainage, splinting, suturing, wound care, and managing infections.
• Analyze patient histories, symptoms, and diagnostic information to develop tailored care plans and maintain compliance with all relevant regulations including HIPAA.
• Actively engage in team meetings and staff development activities to ensure continual improvement.
• Uphold the core values of teamwork, communication, and quality care in every aspect of your role.
• Complete additional tasks as assigned by management to enhance the overall function of the practice.

Patient Population / Client Focus

As a Family Nurse Practitioner at Evergreen Family Practice, you will work with a diverse patient population. Our concierge model allows for a deeper doctor-patient relationship, focusing on preventative care, chronic disease management, and urgent health concerns. Qualifications

• A Master’s Degree in Nursing with specialization as a Family Nurse Practitioner.
• Current Family Nurse Practitioner License demonstrating compliance with state regulations.
• Strong knowledge of policies and operational standards relevant to nursing and healthcare.
• Excellent interpersonal skills, fostering a positive and professional work atmosphere.
• Experience with telehealth platforms and the ability to consult patients remotely.
• Proven ability to thrive in a fast-paced, autonomous setting while ensuring quality patient care.

Preferred Experience

• A minimum of 2 years of experience as a practicing Family Nurse Practitioner in a clinical or primary care setting.
• Experience in concierge medicine or a similar model emphasizing personalized patient care.
• Demonstrated ability to engage with patients through virtual channels, enhancing the overall patient experience.

Compensation and Benefits

We offer a competitive salary range of $125,000 – $140,000 annually, commensurate with experience and skill level.
