Work Environment

This position requires an onsite presence, allowing you to engage directly with patients in their home environments and at community locations. Your scheduled work hours will typically be from 9:00 a.m. to 6:00 p.m., with occasional extended hours into the evening until 8:00 p.m. Participation in weekend community outreach events or health fairs will also be an integral part of your role. You can expect to maintain an average workweek of approximately 40 hours.

Key Responsibilities

• Conduct detailed health assessments and screenings tailored to the unique needs of individuals and families, evaluating both physical and mental health conditions.
• Diagnose and manage acute and chronic health issues, ensuring proper treatment plans are created and followed to promote preventive care and health optimization.
• Collaborate with local community organizations to design and implement health education programs that address prevalent health issues and promote wellness.
• Advocate for patients' access to healthcare services, employing a culturally sensitive approach in all interactions.
• Coordinate referrals and follow-up care, ensuring continuity and quality of care are maintained for all patients.
• Document patient interactions and treatment plans accurately in accordance with regulatory standards to maintain the integrity of medical records.
• Engage actively in community events, providing essential health services and education to underserved populations, thereby raising awareness of health resources available.
• Stay informed about the latest trends in community health and integrate evidence-based practices into your care methods.

This role primarily serves a diverse patient population within the Valley Stream community, including various age groups and backgrounds. Qualifications

• Current Nurse Practitioner certification and active state nursing license are mandatory.
• A Master’s degree in nursing from an accredited institution is required for this role.

Compensation and Benefits
