Work Environment

Located in the vibrant and diverse community of Rosedale, NY, our clinic prides itself on providing a supportive and nurturing work environment. As part of our team, you will engage with patients in an onsite setting, predominantly through house calls, allowing for intimate patient-provider interactions. Expect to work a standard schedule from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending to 8:00 p.m., and you may also participate in weekend community outreach initiatives.

Key Responsibilities

• Conduct comprehensive health assessments and screenings tailored to meet the needs of individuals and families in the Rosedale community.
• Diagnose and manage both acute and chronic conditions, ensuring a focus on preventive care and health promotion.
• Collaborate with local organizations and stakeholders to develop, implement, and evaluate health education initiatives aimed at enhancing community awareness and engagement.
• Provide culturally sensitive care, ensuring that patients' rights are upheld and advocating for their access to necessary healthcare services.
• Effectively coordinate referrals and follow-up care, ensuring patients receive continued support and resources throughout their healthcare journey.
• Document patient interactions and clinical findings meticulously, maintaining accurate medical records in accordance with regulatory standards.
• Participate actively in community outreach events and health fairs, providing valuable health screenings and informational resources to promote overall wellness.
• Stay informed regarding community health trends and best practices in healthcare to continuously enhance service delivery.

Patient Population / Client Focus

This role primarily focuses on serving diverse patient demographics, including underserved individuals and families in Rosedale. By providing holistic care that recognizes the social determinants of health, you will help ensure that all patients can access quality healthcare that meets their unique needs.

Qualifications

• A valid Nurse Practitioner certification with an active state nursing license.
• A Master’s degree in nursing from an accredited institution.
• A minimum of 3-5 years of experience in clinical settings, ideally within community health or public health frameworks.
• Proven strong interpersonal and communication skills, with an ability to engage effectively with diverse populations.

Compensation and Benefits

We offer a competitive salary range of USD 180,000 to 200,000 annually, commensurate with your experience and qualifications.
