Key Responsibilities

• Conduct thorough health assessments and screenings for community members, enabling early diagnosis and intervention.
• Diagnose and manage both acute and chronic medical conditions, ensuring comprehensive preventive care and health promotion.
• Work in collaboration with community organizations and stakeholders to conceptualize, develop, and implement effective health education programs.
• Provide culturally sensitive care that respects the diverse backgrounds of patients and advocates for their rights and access to healthcare services.
• Coordinate patient referrals and follow-up care, ensuring continuity of care and addressing any barriers to treatment.

Qualifications

• Current certification as a Nurse Practitioner (NP) is mandatory.

Compensation and Benefits
